چکیده

A myelomeningocele associated with the ArnoldChiari malformation is now one of the commonest neonatal surgical emergencies. Hydrocephalus used to be the main cause of death in these infants during the first year of life, but since the advent of ventriculo-atrial shunting procedures the mortality and morbidity have improved. However, the management of the hydrocephalus still remains a serious problem. In the past, assessment of the hydrocephalus has depended upon the correlation of the clinical signs such as skull size, separation of the sutures, tension of the anterior fontanelle with the CSF pressure, dye studies, and air ventriculography. More recently radio-isotopes have been used to study the production, circulation, and absorption of the CSF. The aim of the present investigation was to measure the movement of CSF before and after ventriculoatrial shunts in infants with the Arnold-Chiari malformation.

Arnold-Chiari Type II Malformation: A Case Report and Review of Prenatal Sonographic Findings

The Arnold-Chiari malformation is a congenital abnormality of CNS, characterized by downward displacement the parts of the cerebellum, fourth ventricle, pons and medulla oblongata into the spinal canal. This malformation is one of causative factor of death in neonates and infants. A thorough understanding of the direct and indirect sonographic findings is necessary for diagnosis of Chiari II ma...

4D MR imaging of cerebrospinal fluid flow in Chiari I malformation with and without syringomyelia and flow changes after decompressive surgery

Background The aim of our study was to evaluate the feasibility of 4D phase contrast (PC) flow imaging for visualisation and analysis of cerebrospinal fluid (CSF) flow in Chiari I malformation. CSF flow was compared between healthy volunteers and patients with Chiari I malformation with and without syringomyelia. Moreover, the effect of a craniocervical decompression on CSF flow was studied in ...

Computational Investigation of Cerebrospinal Fluid Dynamics in the Posterior Cranial Fossa and Cervical Subarachnoid Space in Patients with Chiari I Malformation

PURPOSE Previous computational fluid dynamics (CFD) studies have demonstrated that the Chiari malformation is associated with abnormal cerebrospinal fluid (CSF) flow in the cervical part of the subarachnoid space (SAS), but the flow in the SAS of the posterior cranial fossa has received little attention. This study extends previous modelling efforts by including the cerebellomedullary cistern, ...

Chiari I Malformation Associated with Atlanto-Occipital Assimilation Presenting as Orthopnea and Cough Syncope: A Case Report and Review of Literature

Although it is not uncommon for patients with Chiari I malformations to present with respiratory complaints, cough syncope is a rare presenting symptom. We report an adult patient who harbored both a Chiari I malformation and atlanto-occipital assimilation who complained of cough syncope, orthopnea, and central sleep apnea. The patient underwent decompressive craniectomy of the posterior fossa ...

A Rare Case of Chiari Type-1 Malformation Accompanied by Symptomatic Cerebrospinal Fluid Hypovolemia: Comparison of Congenital Chiari Type-1 Malformation and Acquired Chiari Malformation Secondary to Cerebrospinal Fluid Hypovolemia: Case Report

A 23-year-old woman was injured in a rear-end collision. She had general malaise and posterior neck pain, which were more severe when she was in an upright position. Magnetic resonance imaging (MRI) revealed the presence of cerebellar tonsil descensus and syringomyelia in the spinal cord. Radioisotope (RI) cisternography showed signs of an early accumulation of RI in the bladder, and a delayed ...

Posterior fossa decompression with tonsillectomy in 104 cases of basilar impression, Chiari malformation and/or syringomyelia.

The prime objective in the surgical treatment of basilar impression (BI), Chiari malformation (CM) and/or syringomyelia (SM) is based on the restoration of the normal cerebrospinal fluid (CSF) dynamics at the craniovertebral junction through the creation of a large artificial cisterna magna. A small suboccipital craniectomy has been emphasized to avoid caudal migration of the hindbrain structur...
